Testimonials from acupuncturists all over the USA:

“I’m so glad I found these forms when I did. I had just passed my boards as an acupuncturist, and with a thousand things to do to start a business, having forms that have already thought of everything at hand made a huge difference. What a lifesaver!” -B.T., LAc

“These forms are a godsend. Everything you need to get started as a new acupuncturist straight out of the box. I am so appreciative of the resources Love for Acupuncturists offers! I am sure I will be buying more of her products in the future. Everything is top quality.” -J.K., LAc

“My clinic has been open for a little over a year now, and I’ve used many of the forms available. Last week, I had a patient arrive who I determined would need a treatment plan of 10 visits. The patient stated they couldn’t afford my proposal. I didn’t have a payment plan form ready, and lo and behold, Love for Acupuncturists did! So easy! What I like about the forms is that they are put together by a practitioner who has been in the game way longer than I have: eliminating the possibility (or probability) that I might forget significant details.” -J.R., DOM, Dipl. OM

“Thanks Laura! These saved me hours and hours of headache!” -G.R., Dipl. OM

“I purchased the package just for the superbill but ended up using so many of the others because they looked so much cleaner than mine.”  -Y.L., LAc

“I just started my practice and these were a lifesaver. Instead of asking around to try to make a new patient information sheet that looked presentable, I now have everything I need to open my practice on July 8th. I love the elimination diet and new patient info one.” -M.B., LAc

“The superbill is compatible with the changing ICD codes. Very useful.”  -T.T., OMD

“Thank you!! I can tell these took a long time to compile.”  -J.M.R., LAc

“Saved me A TON of time when I was opening my practice. They are so clean and professional.” -R.C., LAc

Confused on which format to get?

  • Microsoft Word is by far the most popular bundle, but you must have Microsoft Word installed on your computer (Mac or PC). Microsoft Word is a paid program but does come with some computers.
  • Pages is a FREE program that is only available for Macs.
  • Open Office is also a FREE program which is available for Macs or PCs.

If you have any questions about any of them, contact me!

Disclaimer: Please understand that I am an acupuncturist in California, and although I have been using these forms for 6+ years without any issues, they were created by myself and only myself, not a lawyer or an insurance company. These need to be reviewed by your lawyer and malpractice insurance company so you can make sure they fit with your individual scope of practice laws.

Codes on the superbill are provided as a resource but we make no guarantees about reimbursement for these codes. It depends on many factors including your location, the health insurance carrier, etc.

  1. I was interested in purchasing Microsoft Word Format Forms “Just The Basics” but would like to see a copy of the new patient intake form prior to ordering if that is allowed.
    Thank you,
    danielle
Can I add my logo and information on the top of these forms?

Hi Laura – I am ready to purchase & cannot remember or find your answer to my question, I use apple products, which format will work best?
    Thanks again,

Hi Christy! Either format works fine on a Mac. You just have to have either Microsoft Word or Open Office, both of which can be downloaded on a Mac. Im making a “Pages” version today so that will be available soon as well! Pages is the word processing program that most Macs come with and is free. =)

  3. I’m interested in seeing your intake and pt info forms before deciding if I’m going to purchase. Will you pls send a sample ? If I like those two forms I’m will to pay for the whole packet just to save time revising mine.
    Tkank you

  4. How are we able to purchase the forms if we already have Microsoft word?

    • Laura

The bundle doesn’t come with Microsoft Word, you would just have to own Microsoft Word in order to open and see the forms in the correct format. =) So if you already have Microsoft Word, thats all you need!

    Hi,

I am wondering if I could peek at forms before making decision. Can they be used digitally?

    • Laura
      |

They can be used digitally but they have to be converted in a program that allows for digital forms. You would take the PDF version of the forms and then upload them to a program like https://www.pdfescape.com/ to make them able to be filled out digitally.

  6. Hi Laura,

I have Microsoft Word but I don’t have 2016, is that a problem? I also have a logo an header I would like to add to these forms, How do I do that? Also are these forms that I can do one the computer or do I have to print out the form and handwrite the information on to the form?

    • Laura

Hi Melissa, It shouldnt be a problem to have a different version of Word! You can add a logo and header easily in Word. These are forms for printing and handwriting on, but there are certain programs you can use to convert them into writable PDF forms! =)
